Adapted Update Assessments
for Returning Clients
For returning clients, we want to be sure that we’re up-to-date on what’s been happening with you and also have enough time to orient you to the changes we’ve implemented. We’ll be doing a short, no charge update assessment and orientation to the new regular visit flow with every returning client since we closed our doors back in March. We want to be sure you’re comfortable with how everything works and know what’s expected so that moving forward, your regular visits run smoothly and you can focus on what matters – the experience of your body and healing.
3 tasks before your appointment
1. Online COVID-19 Screening
In order for you to be eligible to make an appointment, you must self-assess for COVID-19. Prior to contacting us to book an appointment, please complete the self-assessment process using our online COVID-19 Screening Form.
2. COVID-19 Update Assessment Form
If possible, please download, print and complete the COVID-19 Update Assessment Form to bring with you when you come. If you don’t have the ability to do so, please let us know and we will make arrangements for you to complete the form in our office.
3. Get to know the new flow
We’re ready to work with you and have prepared a detailed description of the changes to our regular visit process. Please take the time to review our Adapted Regular Chiropractic Visit process prior to your arrival.
Before traveling to our office
Before traveling to our office, please complete a self-assessment process to ensure that you are eligible to attend our office. If you screen positive prior to arrival at our location, we will waive the usual $15 reservation fee for cancellations with less than 24 hours notice.
Upon entry to our office, all clients must be screened for COVID-19.
Clients who screen positive for COVID-19 at our location will have their appointment rescheduled and a $15.00 reservation fee will apply.
